Arizona Youth Partnership (AZYP) in collaboration with Pima Prevention Partnership (PPP) has been
invited to conduct a Teen Maze at your child’s school. AZYP and PPP are non-profit community-based
organizations with offices across Arizona and are participating in this activity through a grant from
the Department of Health and Human Services Administration for Children and Families. Described
as a life-size board game, Teen Maze is an interactive positive youth development activity that helps
youth think about their decision-making and its potential impacts. The development of Teen Maze
will be guided by youth in each community and will focus on helping students envision their future as
it relates to sexual risk avoidance, healthy relationships, goal setting and substance use. At the end
of the Teen Maze, your child may be asked to complete a brief anonymous satisfaction survey to
determine what they thought of the activity.
The purpose of the Teen Maze is to provide your child with information and skills that will enable
them to think about their decisions and the impacts their decisions may have on their lives. This is
considered sexual risk avoidance education and your permission is required. Only sign and return this
form if you DO NOT WISH for your child to participate in the Teen Maze and/or the Teen Maze survey.
No response from you is considered your permission for your child to attend the Teen Maze activity
and complete the Teen Maze survey.
